How to Submit a Takedown Notice
Please provide the following:
- Your full legal name, address, phone number, and email address.
- Identification of the copyrighted work you claim has been infringed.
- Exact URL(s) or location(s) of the allegedly infringing content.
- A statement that you have a good-faith belief the use is not authorized.
- A statement made under penalty of perjury that your notice is accurate and you are the rights holder or authorized to act on their behalf.
- Your physical or electronic signature.

If you believe content was removed in error, you may submit a counter-notice with your contact details, the removed content location, a statement under penalty of perjury that removal was a mistake, and consent to jurisdiction.
